We are a paperless office for our clients files. To help us keep waste low for our planet when you book your appointment an intake form will be emailed to you. Click on the link, complete each page, e-sign the waiver, and "Save" the form, this will submit it to your file in our system. Also, if you were referred by a current client please put their name under "How did you hear about us?". We like to send them a special Thank You!

If you are unable to upload the link that is emailed to you when you book, just come in 5 minutes prior to your appointment to fill out the intake form in the office.
