We are a paperless office for our clients files. To help us keep waste low for our planet when you book your appointment an intake form will be emailed to you. Click on the link, complete each page, e-sign the waiver, and "Save" the form, this will submit it to your file in our system. Also, if you were referred by a current client please put their name on the "Complaints" page under "How did you hear about us?". New Clients will receive a "New Client Package" which will include Price list, Cancellation Policy, Brochure about our different services, and a letter how the company was started.
If you are unable to upload the link that is emailed to you when you book, just come in 5 minutes prior to your appointment to fill out the intake form in the office.
